We are a vital connector between community health workers, local organizations, healthcare systems, managed care organizations, state programs, individuals and families. HCPC is proud to be the first and only Nationally Certified Pathways Community HUB Organization in the state of New Jersey.

Our Mission
Our mission is to significantly improve health and overall quality of life for residents of Northern New Jersey by specifically addressing social drivers of health through a dedicated, thriving and sustainable community coalition and care hub.
Our Stories
Coordinating Complex Care
Maria, 53, was battling diabetes and facing eviction, all while trying to manage her complex health needs. With support from HCPC’s Community Health Workers, she received coordinated medical care and connection to housing support. Today, she’s stable in her home and managing her health effectively.
Restoring Hope and Sight
George, 39, was in a desperate situation – legally blind, unemployed, without health coverage, and living alone. Then he got connected to an HCPC Community Health Worker who helped him secure Medicaid and attended his medical appointments. After four months, George regained 80% of his vision and was able to rebuild his life
Surrounding Seniors with Support
Frank and Maria had always valued their independence. But at 70 and 69, they faced mounting anxiety as healthcare and grocery costs kept rising. Through HCPC’s Community Health Worker, they accessed Medicaid benefits and connected with a local food pantry. “We’re still very independent,” Maria says, “but as we age, we know how important it is to have a support system.”
